Warrior of Wellness Program
The Warriors of Wellness (WOW) project serves the Portland metro area and brings together a variety of community organizations that employ Community Health Workers, along with health systems and insurers. The goal of the project is to build a model where Community Health Workers can provide culturally-specific services to Medicaid members by contracting with health systems as a community collaborative. Community Health Workers involved in the WOW project work with community members who access services through the social service agencies where they are housed, and provide education, information and support to help individuals and communities to meet their health goals.
