Reports to: Executive Director
Hours: 40 hours weekly (some evenings and weekends)
Pay: $17.50 - $21.58 (based on experience and education)

Position Summary
The Case Manager is responsible for assessing the needs of our guest families, creating household stabilization plans and developing/maintaining relationships with agencies and community organizations to help our guests regain and retain housing. The work week is divided between our Day Center in Hillsboro, our family housing site and working remotely from home. A strong ability to work autonomously is required.
The successful candidate will have strong Spanish speaking skills, proven counseling and case management expertise and a passion for working with people who need a “hand up”. Strong computer proficiency with Microsoft Office products as well as Google documents is required.

Family Promise of Washington County (FPWC) helps families and Adult only households experiencing homelessness achieve sustainable housing and independence through a community-based response by providing shelter, intensive case management and supports while our guests work towards re-housing. Our vision is a community in which every family has a home, a livelihood, and a chance to build a better future. Our small but mighty team is looking for our new team member.
Primary Duties & Responsibilities:
Case Management
• Conduct intake interviews and screen the needs of guests. Determine if they are good candidates for the FPWC program. If so, enroll into FPWC and if not, provide resources and supports for additional community programming.
• Assist school district staff in evaluating housing needs of students and their families identified as homeless.
• Develop and maintain relationships and collaboration with local social service agencies, property managers, and employers.
• Orient guests on FPWC protocols and responsibilities at our emergency response sites.
• Develop individual family case plans with the goal of securing sustainable housing and employment.
⦁ Identify tasks that will help the guests reach their goals and empower them to succeed.
• Assist guests with social service needs, referring to the proper agencies, and advocating with agencies as required.
• Support families as they advocate for their children at the school level.
• Schedule guest program training as necessary, i.e., financial literacy, parenting skills, etc.
• Meet with guests two to three times per week to monitor progress. Maintain confidential
records of guests’ needs and progress.
• Maintain confidential records of guest interactions and input guest data and statistics into HMIS and FPWC databases.
• Participate in weekly case conferencing with FPWC team. Review guest progress with Executive Director and provide reporting as necessary.
• Provide up to one-year of post-graduation support to program families.
Day Center Facility
• Share in operational tasks of the Hillsboro Day Center, i.e., opening and closing, cleaning, telephone coverage, etc.
• Participate in staff meetings, meetings with Host and Volunteer Coordinators, training sessions, and special events as required.
• Represent FPWC at community meetings and events when requested.
• Help to provide hospitality as needed to guests.
• Assist Executive Director as needed.
Education and Experience Desired
• Three years of applicable work experience with a preference to those candidates with a Master’s in social work preferred or a Bachelor’s in social work, psychology or related human services field and two years of applicable work experience required.
• Experience working in a social service agency with case management experience working with adults and children is preferred.
• Experience working with homeless families highly desirable.
Skills Required
• Resourceful and tenacious with an ability to help break down barriers to help our guests reach success.
Strong computer skills and ability to learn computer programs and systems and operate Microsoft Office Suite programs and Google based programs.
• Knowledge of local government and social service programs is desirable.
• Excellent communication skills and ability to work autonomously.
• Good planning and organization skills coupled with flexibility.
• Ability to handle and prioritize multiple tasks and work through distractions.
• Proven counseling/problem solving skills and ability to work effectively with a wide range of
people; ability to develop a comfortable, trusting relationship with guest families.
• Ability to maintain confidentiality expectations of the position.
